Reframing Perioperative Clinical Competence in Nursing Education: toward a Systems-Integrated and Experience-Driven Paradigm

Introduction: Perioperative nursing competence is fundamental to ensuring patient safety, high-quality surgical care, and effective interprofessional collaboration. However, undergraduate nursing education continues to rely heavily on reductionist competency frameworks that emphasize checklist-based performance and self-reported readiness. While these approaches provide standardized assessment, they inadequately capture the dynamic, contextual, and relational nature of perioperative clinical competence. Consequently, there is a growing need to reconceptualize competence beyond isolated technical skills toward an integrated understanding that reflects authentic clinical practice.
Methods: This study employed an integrative critical narrative review guided by the Scale for the Assessment of Narrative Review Articles (SANRA) to ensure methodological rigor and transparency. Electronic searches were conducted in PubMed, Scopus, CINAHL, Web of Science, and Google Scholar for literature published between January 2010 and May 2026, supplemented by seminal theoretical works published before 2010. Database searches initially identified 842 records, of which 176 duplicates were removed. Following title and abstract screening, 214 full-text articles were assessed for eligibility. After applying predefined inclusion and exclusion criteria, 67 publications were included in the final synthesis. Data were analyzed using an iterative thematic synthesis that identified recurrent conceptual patterns across theoretical, empirical, systematic review, and policy literature.
Results: Five interconnected themes emerged from the synthesis: (1) limitations of reductionist competency assessment and self-evaluation; (2) contributions and limitations of simulation-based education; (3) sociocultural influences and the hidden curriculum; (4) experiential learning and reflective practice; and (5) competence as a complex adaptive system. Collectively, the evidence demonstrates that perioperative competence is not a static achievement but an evolving capability arising through authentic clinical engagement, interdisciplinary collaboration, contextual learning, and continuous reflection.
Conclusion: This review proposes a Systems-Integrated and Experience-Driven Framework for Perioperative Clinical Competence, which extends existing competency models by integrating experiential learning, systems thinking, sociocultural theory, and authentic assessment into a unified conceptual model. Unlike traditional competency frameworks that primarily evaluate observable task performance, the proposed framework conceptualizes competence as an emergent, adaptive, and relational phenomenon. The framework provides a theoretical foundation for curriculum innovation, authentic competency assessment, and future empirical validation within undergraduate nursing education
